Transportation Options

All transportation must be preceded by a medical clearance and a program-confirmed date and time of admission.

Door-to-door transportation is required; patients must admit directly from where they were last medically cleared, or the medical clearance will need to be restarted before admission can be completed.

Transportation is preferred to be provided by the transferring/referral entity, or a transportation program provided through the transferring/referral entity.

Transportation may be provided by family members, sponsors, members of the patients’ support network, or formal services, such as Uber, Lyft, or a taxi service; all must verify upon arrival that no stops were made during transportation.
